Imaging records are available through this easy to use service. Instead of picking up a CD, you can access your records from the comfort of your home.

- You can instantly share these records with any doctor or family member.
How to get Clearpath
- Simply Create an Account at https://patient.myclearpath.com/login.
- You will be asked to enter in your First and Last Name, Date of Birth, E-mail, and Time zone.
- Once you create an account, you will get a temporary password sent to your e-mail.
- Use the temporary password and your email address to log in.
- Request your records.
* Prior to requesting your records, please ensure a copy of your Photo ID is attached to your profile under the My Identity Tab.
How to Request Records
- Once logged into Clearpath, select the Request Record Tab.
- To request a record, select the Request Record button on the bottom right corner.
- Select Record Custodian (Provider Location).
- If you do not see the custodian name listed, please reach out to help@myclearpath.com.
- Select records to request, Body Part, and date Range.
- Agree to the terms and conditions and submit.
*Once submitted, facility will review and process. Once request is processed, if approved, you will see your images under the My Records Tab.
